Applying for a Raymond Wentz Foundation Grant
Welcome to the Raymond Wentz application process. If you or a loved one is in financial crisis as a result of a cancer diagnosis, and thus are not able to keep up with basic living expenses like rent, groceries, utility bills, and medical co-payments, then you may qualify for a grant. All applicants must be referred through their doctor, social worker, nurse or patient navigator. Applications are collected all month long and on the 2nd Monday of the following month, qualifying applications are reviewed by the gifting committee. Once the committee has approved or denied the request, the applicant is notified.
GRANT GUIDELINES:
- Grant applicants must have a cancer diagnosis and be receiving active cancer treatment.
- Grant applicants must be Colorado residents.
- Grant applicants or Parent/Legal Guardian of Grant applicants must enclose copy of identification.
- Grant applicants may submit one request per year for a maximum of two lifetime awards
